
Interim & Final Narrative & Financial Report Guidelines
Reporting Period: An interim report that includes information on project activities and financial expenditures (see specific requirements detailed below) is due seven months after the start of the grant (July 31 or January 31, respectively). A final report fulfilling the same requirements is due one month after the grant period concludes.
This narrative report serves as an opportunity for you to detail your program activities and outcomes during your grant period. Please be candid and detailed in your responses, even if you were aiming for better results; we understand that any struggles you may have experienced are part of the challenges of your organizations work, and learning about them will be very informative to our efforts at a national level.
In your report, please answer each of the following questions clearly and concisely.
- Describe any fundamental changes to the project since the grant start date (such as service cuts or expansion, major staffing changes, etc.).
- How has the budget of your syringe access program changed compared to when you submitted your application?
- Describe your organizations accomplishments since the beginning of the grant period that you credit in part or in full to the support you received from The Comer Foundation.
- Total number of syringe access clients served during the grant period (indicate duplicated or unduplicated). If unduplicated, please indicate how you uniquely identify clients.
- Any demographic data you have on clients served.
- Total number of syringes exchanged or distributed during the grant period. Please indicate if any of these were through peer or secondary exchange.
- What challenges have you experienced during the grant period?
- Restate the goals and objectives in your original proposal below. Is your organization on track to reach the goals and objectives by the end of the grant period?
- Describe how the public policy environment in your state or locality has helped or hindered your work.
- Describe what technical assistance needs your organization may have, either at an organizational, program or policy level. Do you currently have access to the type of technical support you need to be successful? Please explain.
- Provide any additional information you would like to share with The Comer Foundation. Please include any compelling stories that illustrate the significance of your work.
Please provide the following in addition to your narrative response:
In addition to providing the narrative information detailed above, please provide a financial accounting of your expenditures. Provide a line-item budget for your project detailing expended funds.
